Coming From Home

Coming from your home or apartment into assisted living is a big life change.  We are here to guide you through the process and make sure the transition is as smooth as possible.  Let’s look at some of the steps that we will be taking once we have found the perfect place.

Documentation

Having the documentation prepared ahead of time will help to make the day of move in an easy process for both you and your loved one.  Download our checklist of what will be required for each step.

Doctor Visit

Many times a visit to the primary care doctor will be needed to have them complete portions of the residency agreement we have received from the selected location. Having a current physician signed medication list and documentation of being free from TB that has been read clear and signed by a medical professional is required by all assisted living settings.

Readying the Room

Bringing in furniture, pictures, or anything familiar is highly encouraged.  This should be thought of as their own space to be made as comfortable as possible.  If you require assistance with moving items or setting up the room, we have companies that are willing to help!  

Move in Day

Transportation to the new home can be done by family, cab, or medical transportation company.  Please ask your representative for recommendations if you require medical transportation or go to our Resources page to do your own search!  Bring with you all of the documentation on the checklist and any toiletries you may need. 

From Hospital or Skilled Nursing

When you or your loved one is coming from a hospital or skilled nursing facility the medical documentation needed to move into assisted living is already done for us.  We will gather the appropriate forms and make sure the transition into the new place is seamless. 

Documentation

We work directly with the case management department at the hospital or skilled nursing facility so you you can focus on your loved one.  We will coordinate with the medical staff and make sure all the proper documentation that the new home or center will requires for moving in is in order and prepared for move in day.
